HOW TO GET A COPY OF A SOCIAL SECURITY CARD FROM THE SOCIAL SECURITY ADMINISTRATION OFFICE


 

What do I need to bring to the Social Security Administration office to obtain a copy of a social security card?

For an individual to get a copy of a social security card for a child, they have gained the custody of the individual through the courts. When going into the social security office and requesting a copy of an individual’s social security card one must have the following documents with them to show they are entitled to the social security card for that person.

  • Having a court order stating the legal right of custody to the person who they are requesting a social security card the court custody document must have the original authentication from the judge with the courthouse stamp perforation completely visible. Bring original document but, keep the original document for your records.

  • The Social Security Administration office will accept a medical document with the child’s name, birth date signed by the doctor and stamped by the doctor’s office or hospital. The medical report must not be older than two years old.

  • Or bring a school document with the name and birth date signed by the school’s administer. The school document from the school's executive administrator must not be older than two years old.

  • Please note that the only time the Social Security Administration will look at a birth certificate is when there is an amendment to the name, birth date of the child or adoptive parents’ have a name change.

  • The individual needing the social security card can have school identification or a medical card, but the school or medical card must have an effective date.

  • The individual requesting the information from the Social Security Administration must bring current identification for their selves such as current state license not expired. Or current military identification or passport not expired, or they can bring current Veteran Administration identification not expired. Must stress the importance of current ID no expired identification will be accepted.
